The Downer Cow — When She Cannot Rise, and Every Hour Counts Twice

A cow down and unable to rise is dairy's most time-critical picture — partly for what put her down, and equally for what lying itself does next: a heavy body crushes its own muscles and nerves within hours. Whatever the cause, downer care runs on two clocks — the doctor's diagnosis, and your bedding-and-turning nursing.
Common Signs
- Unable to rise despite genuine attempts — or not attempting at all
- The context points at causes: fresh calver (milk fever first suspect), high fever (infections), after slipping/trauma (injury), stiff in insect season (three-day sickness)
- Cold ears and skin in the classic calcium collapse; hot fever in infective causes
- Appetite and alertness ranging from bright-but-stuck to profoundly dull
- The secondary enemy appearing with time: pressure damage to the downside legs
Possible Causes
- Milk fever around calving — the most treatable, most urgent classic
- Calving injuries and nerve damage from a hard pull
- Slips and falls on wet floors; fractures and hip damage
- Severe illness of any kind: toxic mastitis, HS, acute infections
- After ANY initial cause: pressure damage from the lying itself — the downer spiral
What the Farmer Should Check
- Context first for the doctor's phone call: calved when? Fever? Injury event? Eating?
- Ears and skin cold (calcium picture) or burning (fever picture)?
- Can she sit up chest-wise (better) or is she flat on her side (worse — prop her up NOW)
- Surface: is she on bedding or bare concrete/mud?
- Bladder and dung passing? Note it — the doctor will ask
Feed Guidance
Call the veterinary doctor immediately — downer causes from milk fever to infection are treatable mainly in their first hours — and while waiting, nurse like it matters, because it does: get her OFF bare concrete onto deep bedding, prop her chest-up with straw bales or sacks (flat-on-side risks bloat and inhaling cud — never leave her flat), and TURN her from one hip to the other every few hours around the clock; this turning is the single act that prevents the pressure damage which kills downer cows after their original disease was cured. Keep lukewarm water and soft tempting feed within neck reach, shade or warmth per season, and firm footing (dry bedding, not slick muck) for her rising attempts. Once she stands — help her keep standing awhile, expect wobbliness, and rebuild feeding gently under the doctor's guidance.

When to Contact a Veterinary Doctor
Every downer cow is a same-hours veterinary emergency — and if she remains down after initial treatment, ongoing veterinary review plus disciplined turning-and-bedding nursing decide the outcome. Lifting devices and slings exist; dragging a downer animal does not belong on that list.
